La hemorragia subaracnoidea por aneurisma (HSAa) se asocia a una lesión cerebral temprana y retardada debido a varios procesos subyacentes e interrelacionados, que incluyen la inflamación, el estrés oxidativo, el endotelio y la apoptosis neuronal. La derivación cardiopulmonar (CEC) es una técnica estándar para la cirugía cardíaca, pero conlleva el riesgo de complicaciones neurológicas graves (por ejemplo, ictus) causadas por embolias y/o reducción de la perfusión cerebral. Presentamos un prototipo de cánula aórtica (optiCAN) con flujo de salida helicoidal y punta de dispersión de chorro que podría reducir el riesgo de embolias y restablecer la perfusión cerebral en un 97,5% del flujo fisiológico durante la CEC in vivo, mientras que una cánula comercial de punta curva produce un 74,6%. En otra comparación in vitro, los parámetros de pérdida de presión y hemólisis de optiCAN no se ven afectados. Los resultados se confirman de forma reproducible in silico para una anatomía aórtica humana ejemplar mediante simulaciones de dinámica de fluidos computacional (CFD). Basándonos en las simulaciones CFD, mostramos en primer lugar que el diseño de optiCAN mejora el lavado de la raíz aórtica, lo que reduce el riesgo de tromboembolismo. En segundo lugar, identificamos las regiones de la íntima aórtica con mayor riesgo de desprendimiento de la placa mediante la correlación de las zonas de mayor crecimiento de la placa y las tensiones de cizallamiento de la pared (WSS) elevadas. A partir de esto, proponemos otro diseño de cánula fácil de fabricar (opti2CAN) que disminuye las zonas con una carga de WSS elevada, a la vez que preserva el flujo cerebral fisiológico y una hemodinámica favorable. Con este novedoso diseño de cánula, proponemos una opción de canulación para reducir las complicaciones neurológicas y la prevalencia de accidentes cerebrovasculares en pacientes de alto riesgo tras la CEC.
